Cost guide
AI receptionist cost: per-minute platforms, packaged plans and custom builds
AI receptionist cost depends on which of three routes you take: a per-minute voice platform you build on, a packaged receptionist with a monthly plan, or a custom build that connects to your calendar, CRM and phone system. This guide is for office managers, ops leads and IT owners comparing quotes for answering calls, booking appointments and transferring callers to staff. It sets published vendor prices side by side and lists the running and compliance costs that headline rates leave out.

Quick answer
How much does an AI receptionist cost per minute?
AI receptionist cost on voice platforms runs from about $0.05 to $0.14 per minute on published vendor pricing: Vapi lists $0.05 plus model, speech and telephony, Retell AI $0.055 (about $0.11 all-in in its example), ElevenLabs Agents $0.08 and Bland AI $0.14 all-inclusive. Packaged plans such as Smith.ai's start at $150 per month.
- Headline per-minute rates often exclude model, speech and telephony charges.
- Packaged receptionists price per month or per call; platforms price per minute.
- A custom build adds a one-time cost but connects to your scheduling and CRM.
- HIPAA, consent and call recording rules can add fees and change vendor choice.

How much does an AI receptionist cost per minute?
Published platform rates sit between about five and fourteen cents a minute, but they do not all cover the same things. Some vendors quote only their own platform fee and pass through the language model, speech recognition, voice synthesis and phone line costs separately, while others bundle everything into one rate.
That makes the all-in figure the one to compare. Retell AI is helpful here because its pricing page shows an example that totals about $0.11 per minute once the other components are added to its $0.055 platform fee. Bland AI's $0.14 per minute on its Start plan is quoted as all-inclusive, so the two are closer than the headline numbers suggest.
Volume also changes which rate matters. A small office with a few hundred minutes a month will barely notice a few cents of difference per minute, so integrations, call quality and transfer handling should drive the choice. At several thousand minutes a month, the gap between a platform-only rate and an all-in rate starts to show up clearly on the invoice.
How do per-minute platforms, packaged receptionists and custom builds compare?
Each route trades setup effort for control. The table lists published figures from each vendor's pricing page as of October 2026 (and one reported range); they are third-party prices, not ours, and they change often, so check the source before budgeting.
| Option | Source | Published price | What to check |
|---|---|---|---|
| Per-minute platform | Vapi pricing page | $0.05/min platform | Model, speech and telephony billed on top |
| Per-minute platform | Retell AI pricing page | $0.055/min platform; about $0.11/min all-in in its example | Which model and voice the example assumes |
| Per-minute platform | ElevenLabs Agents pricing page | $0.08/min | What the rate covers for your call volume |
| Per-minute platform | Bland AI pricing page | $0.14/min all-inclusive on its Start plan | BAA only on its Enterprise plan |
| Packaged receptionist | Smith.ai | AI receptionist Pro $150/month (about $2 per call) | How calls beyond the plan are billed |
| Packaged receptionist | Simple off-the-shelf tools (reported) | Roughly $49-99/month | Integrations, call transfer and HIPAA terms |
| Custom build | Your chosen platform plus a build partner | One-time build, then platform, model, speech and telephony at vendor rates | Who owns the accounts, prompts and call logs |
What are the running costs of a custom AI receptionist?
A custom receptionist has four running cost components, and each one is billed by usage. Knowing them separately helps you see which lever to pull if the bill climbs.
Two design choices move these components most. Shorter, well-scripted turns reduce speech and model usage per call, and a clear rule for when to transfer to a person stops the agent from spending minutes on calls it cannot resolve. We set both during the build and check them against real transcripts in the first weeks.
- Platform: the voice agent platform's own per-minute fee (Vapi, Retell AI, ElevenLabs Agents or Bland AI), which runs the call logic, turn-taking and tool calls.
- Model: the language model that understands the caller and decides what to do next, from OpenAI, Claude, Gemini or another provider, billed by tokens or folded into the platform rate.
- Speech: speech-to-text that transcribes the caller and text-to-speech that produces the agent's voice, each usually priced per minute or per character.
- Telephony: the phone number and carrier minutes, often a Twilio number, plus any minutes spent on warm transfers to staff.
How do you estimate a monthly AI receptionist cost?
Multiply your monthly call count by the average call length, then by the all-in per-minute rate, and add fixed items such as phone number rental and compliance add-ons. Your phone system's call reports usually give both the call count and the average duration.
As a worked example only: a practice taking 600 calls a month at an average of three minutes uses 1,800 minutes. At Retell AI's example all-in rate of about $0.11 per minute, that is roughly $198 a month in usage, before number rental. On Bland AI's all-inclusive $0.14 rate, the same minutes come to about $252.
Packaged products price differently. Smith.ai describes its $150 Pro plan as about $2 per call, so its cost tracks the number of calls rather than their length. Per-call pricing tends to suit offices with long calls; per-minute pricing tends to suit offices with many short ones. Run both calculations on your own call report before deciding.
Which compliance costs belong in the budget?
If calls involve protected health information, the HIPAA requirement for a signed Business Associate Agreement is the largest compliance cost item. Any AI vendor handling that information needs a BAA, and major model vendors sign them only on certain API or enterprise tiers, not consumer plans. Vapi sells HIPAA compliance as a $2,000 per month add-on, and Bland AI includes a BAA on its Enterprise plan, so a healthcare deployment can cost far more than the per-minute rate implies.
Consent and disclosure add design work rather than a line item, but they still cost time. The FCC ruled on February 8, 2024 that AI-generated voices count as artificial voices under the TCPA, so outbound calls using them need prior express consent. Some states require a caller to be told they are speaking with an AI (Utah's AI Policy Act and California's bot disclosure law), and states with two-party consent rules for recording apply to AI receptionists that record calls. This is not legal advice; check your situation with counsel.
- BAA-eligible tiers for the voice platform and the model provider.
- HIPAA add-on fees, such as Vapi's $2,000 per month.
- Consent records for any outbound AI calls.
- A greeting that discloses the AI and any call recording.
When is a packaged receptionist cheaper than a custom build?
A packaged product usually wins when call volume is low and the job is simple: take a message, answer hours and directions, and book into a calendar the product already supports. There is no build cost, and a monthly plan is easy to cancel.
A custom build starts to pay off when the receptionist has to work inside your systems. Typical reasons are booking into a practice management system or EHR, creating or updating records in HubSpot or Salesforce, routing by location, language or caller history, handling regulated data under a BAA, or running enough minutes that you want to choose each component yourself.
What extra costs appear after launch?
The extra costs are mostly people's time. Someone should review a sample of calls each week, especially failed bookings, abandoned calls and transfers, and the prompt or routing rules get adjusted from what they find.
Plan for call recording and transcript storage, number porting if you move an existing line, and updates when the model vendor changes its lineup. In our builds, every call is logged with its transcript, the tools the agent called and the outcome, and a cost cap per call stops a stuck conversation from running up minutes.
Integration upkeep is the other recurring item. If the receptionist books into a calendar or writes to a CRM, an API change or a renamed field on that side can break bookings silently, so we add a daily check that a test booking succeeds and alert someone when it does not.
How do we price an AI receptionist build?
We quote a fixed price after a free review of your call types, phone system, calendar and CRM. The review produces a call flow (greeting, intent detection, booking or message, transfer rules, after-hours handling) and a usage estimate based on your own call reports.
The platform, telephony and model accounts are opened in your name, so the per-minute charges go straight to you at vendor rates, and you keep the prompts, configuration and call logs. A person remains the fallback for anything involving payments, complaints or clinical questions. For the broader picture of build and running costs, see our AI implementation cost guide.

FAQ
Frequently asked questions
What is the cheapest AI receptionist?
Simple off-the-shelf AI receptionists are reported at roughly $49-99 per month, and they suit basic message taking and booking. The cheapest option on paper can cost more if it lacks call transfer, the integrations you need, or HIPAA terms, so compare features against your call types before price.
Do AI receptionist per-minute prices include the phone line?
Not always. Vapi's $0.05 per minute covers its platform, with model, speech and telephony billed on top, while Bland AI quotes $0.14 per minute as all-inclusive on its Start plan. Always ask for the all-in rate per minute and whether the phone number is extra.
Is an AI receptionist HIPAA compliant?
Only if every vendor that handles protected health information signs a Business Associate Agreement. Major model vendors sign BAAs only on certain API or enterprise tiers, Vapi sells HIPAA compliance as a $2,000 per month add-on, and Bland AI includes a BAA on its Enterprise plan.
Can an AI receptionist make outbound calls legally?
The FCC ruled on February 8, 2024 that AI-generated voices are artificial voices under the TCPA, so outbound calls using them need prior express consent. Some states also require disclosure that the caller is an AI. This is not legal advice; confirm your use case with counsel.
Is an AI receptionist cheaper than a human answering service?
For high volumes of routine calls it often is, because usage pricing scales with minutes rather than staff hours. The comparison depends on your call length, the share of calls that still need a person, and compliance add-ons, so model it with your own call report.
How long does it take to set up a custom AI receptionist?
It depends on the number of call types and systems involved. A receptionist that answers common questions, takes messages and transfers calls is far quicker to build than one that books into an EHR and updates a CRM. We fix the timeline alongside the price after a free review.
